Primary Care/Chronic Care Follow-up

Service Description

This will be a shorter visit than your initial appointment to manage chronic health conditions. Typically we will meet virtually every 3-6 months (some conditions may require more frequent visits). At this visit, we will discuss your treatment plan, how your symptoms have been managed, and how you've felt since your last visit. You will be evaluated by one of our experienced providers and together you and the provider will decide to continue or adjust your care plan as needed to help you reach and maintain all your health goals. Prescription medications can be managed as needed. Examples of conditions treated (not all conditions listed): • Uncomplicated DM Type 2 Management • Hypertension • Asthma • COPD • Anxiety/Depression • Hypothyroidism/Thyroid Issues
